Job Details:

  • Role: Part-Time Nurse Practitioner
    • A minimum of 2 days/week preferred, with flexibility to work Sunday through Saturday.
  • Responsibilities:
    • Conduct comprehensive in-home health risk assessments for our members.
  • Compensation:
    • $90 per completed visit
    • $25 no-show fees
    • Mileage reimbursement at the federal rate after the first 35 miles traveled daily
  • Program Overview:
    • Our program is designed to provide in-home health risk assessments to our members, with a focus on delivering high-quality care and improving health outcomes.
  • Location:
    • New Hampshire (with a focus on the following counties: Rockingham, Hillsborough, Cheshire, Merrimack, Strafford, Carroll, and Grafton)

Requirements:

  • Current licensure as a Nurse Practitioner in the state of New Hampshire
  • Minimum of 2 years of experience as a Nurse Practitioner
  • Ability to work independently and as part of a team
