Key Responsibilities:
- Deliver skilled nursing care to patients with complex needs, including wound care, medication management, and symptom control.
- Conduct thorough assessments of patients' physical, emotional, and social needs, and develop personalized care plans to address these needs.
- Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive care and coordination.
- Provide education and support to patients and their families, empowering them to make informed decisions about their care.
- Document patient progress, including medical history, treatment plans, and outcomes, to ensure accurate and timely communication with healthcare teams.
What We Offer:
- A competitive salary and benefits package, including a $20,000 sign-on bonus and up to $20,000 in loan forgiveness for eligible full-time positions.
