Key Responsibilities- Plan and deliver visit content and schedule based on client capabilities, condition, and preferences, as well as physician orders and available resources.
- Maintain a working knowledge of applicable laws and regulations, as well as agency policies and procedures, and adhere to them in a manner that reflects honest, ethical, and professional behavior.
- Perform specific treatments and administer medications to implement the medical and nursing care plans.
- Apply appropriate preventive, therapeutic, and rehabilitative nursing procedures and techniques to clients.
- Utilize interview, observation, and evaluation in assessing clients and apply nursing judgment, consistent with practice standards, in making recommendations to the primary nurse and client.
- Graduate from an accredited or state-approved school of practical nursing.
- Current licensure in the state of Connecticut.
- Minimum of one (1) year experience in an acute care setting.
- Home healthcare experience preferred.
- Interpersonal skills sufficient to interact effectively with clients and their families, as well as all interdisciplinary caregivers, peers, subordinates, and supervisors.
- Passing of National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.
- Current Driver's license and reliable transportation to and from work site.
