Job Description
We are seeking a compassionate Registered Nurse to join our team. As a Registered Nurse with a focus on hospice and palliative care, you will evaluate patients, create personalized care plans, and ensure seamless communication among the patient, family, and care team. Your commitment to providing exceptional care and meaningful experiences is essential in supporting patients facing life-limiting illnesses.
Responsibilities
- Evaluate patients and create care plans tailored to their unique needs
- Communicate effectively with patients, families, and care teams to ensure comprehensive care
- Provide compassionate care and support to patients and their families
Qualifications
- Graduate of an accredited school of nursing with a current state license as a registered nurse
- 2+ years of nursing experience in a clinical care setting (hospice experience preferred)
- Reliable transportation and ability to sit, stand, bend, move intermittently, and lift at least 80-100 lbs
