Job Description

We are seeking a highly motivated Mental Health Service Coordinator to join our team at Friend Health. In this role, you will work collaboratively with individuals, families, and healthcare providers to coordinate services and ensure seamless care delivery.

About the Role
You will be responsible for developing and implementing individualized treatment plans, coordinating services with community-based organizations, and providing direct support services to clients. You will also work closely with interdisciplinary teams to ensure effective service coordination and delivery.

Requirements
To be successful in this role, you will need a Bachelor's degree in a related field, such as social work or psychology. You must also have valid driver's license and vehicle to travel to different locations. Additionally, you should have knowledge of social services and medical needs of psychiatrically disabled persons, as well as sensitivity to multicultural differences.

Work Environment
This is a full-time position working in a typical office environment. You may be required to spend extended periods standing or walking, and may lift up to 15-25 pounds. We offer a competitive salary range of $70,000 - $90,000 per year, depending on experience and qualifications.
