About the Role

Aveanna Healthcare is seeking a skilled and compassionate Licensed Practical Nurse (LPN/LVN) to join our team as a Private Duty Nurse. As a Private Duty Nurse, you will provide one-on-one care to our patients in the comfort of their own homes, assisting them with transitioning home from the hospital and providing the continuous care needed to remain at home with their loved ones.

Responsibilities
  • Implement and coordinate care plans in accordance with physician orders
  • Conduct patient assessments and coordinate care to achieve expected outcomes
  • Administer prescribed medication, treatments, and therapies as needed
  • Document all activities, assessments, and nursing actions in a timely manner
  • Provide education and support to patients and their families on disease management, self-care techniques, and prevention strategies
Requirements
  • Current, unrestricted state license as a Licensed Nurse in the state of practice
  • Current CPR certification
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures
