Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Home Health Registered Nurse. As a member of our team, you will have the opportunity to make a meaningful impact on the lives of our patients and their families by providing high-quality, patient-centered care in the comfort of their own homes.

Responsibilities
  • Coordinates and oversees all aspects of patients' home care, including developing and implementing care plans, educating families on patient condition and needs, and performing head-to-toe physical assessments.
  • Collaborates with patient care teams to ensure seamless care delivery and communicates patient progress to the clinical manager, physician, and care team.
  • Supervises LPNs and HHAs, ensuring that they are providing high-quality care to our patients.
  • Completes documentation timely and accurately, ensuring that all necessary information is recorded and available for future reference.
Requirements
  • One year of clinical experience as a registered nurse (RN) is required, with a minimum of six months of home health experience.
  • Current RN license, specific to the state(s) you are assigned to work, is required.
  • CPR certification and a valid driver's license are also required.
  • Able to assess patient status and identify requirements relative to age-specific needs.
