At RELODE, we are committed to delivering innovative solutions for senior care delivery, focusing on prevention and care coordination. We are seeking a skilled Advanced Practice Provider to join our team in providing Provider-level direct patient care services within post-acute care settings.
Key Responsibilities- Embody our company's care philosophy to enhance each patient's clinical status and quality of life.
- Conduct Annual Wellness Visits and other clinical assessments as needed.
- Provide ongoing evaluation and treatment of chronic illnesses.
- Collaborate with physicians and other healthcare professionals for comprehensive medical management.
- Maintain communication with families and attend conferences when possible.
- Must hold a valid nurse practitioner or physician assistant license and be board-certified.
- Bachelor's and Master's degrees in Nursing from accredited institutions.
- Nursing experience is required, with some preference for Nurse Practitioner/Provider experience.
- Strong leadership, interpersonal, and problem-solving skills are preferred.
- Excellent written and oral communication skills are necessary.
- Punctuality, organizational skills, and teamwork abilities are essential.
All employees must be fully vaccinated on their first day of employment, complying with EEOC guidance on vaccines and exemptions.
