As a RN Case Manager, you will play a vital role in providing patient-centered care and collaborating with the care team to develop and implement a plan of care.
Responsibilities- Conduct Patient Assessments: Perform thorough assessments of patients to identify their needs and develop a plan of care that meets their unique requirements.
- Collaborate with the Care Team: Work closely with the care team, including physicians, therapists, and other healthcare professionals, to ensure that patients receive comprehensive and coordinated care.
- Teach and Educate Patients: Promote patient health and independence by teaching and educating patients on self-care techniques and providing guidance on how to manage their conditions.
- Supervise LPNs and HHAs: Supervise and mentor Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs) to ensure that they are providing high-quality care to patients.
- Document Patient Progress: Accurately and timely document patient progress, including any changes in their condition or response to treatment.
- Communicate with the Clinical Manager: Regularly communicate patient progress to the clinical manager and care team to ensure that everyone is informed and aligned.
- Participate in Clinical Development: Participate in clinical development and continuing education programs to stay up-to-date on the latest best practices and research in home health and hospice care.
- One Year of Clinical Experience: One year of clinical experience as a Registered Nurse (RN) is required. If less than one year of clinical experience as a RN, candidate must be approved by the VP Clinical.
- Current RN License: Current RN license, specific to the state(s) you are assigned to work, is required.
- CPR Certification: Current CPR certification is required.
- Valid Driver's License: Valid driver's license, reliable transportation, and liability insurance are required.
