Position Overview:
As a Clinical Care Coordinator in the Neuro-Oncology department, you will collaborate with a multidisciplinary team to manage care for patients with complex neurological conditions.
Key Responsibilities:
• Collaborate with the care team to identify patients requiring ongoing care coordination for their neurological conditions.
• Assess and determine the level of involvement needed from the care team and coordinate with primary care providers.
• Utilize assessment tools to identify patients with potential care needs that necessitate coordination.
• Conduct outreach to high-risk patients to ensure timely and effective care delivery across various settings.
• Manage patient populations using technological tools, including registries and patient lists.
• Perform comprehensive clinical assessments addressing medical, behavioral, and social needs.
• Communicate care coordination plans to patients and families, ensuring all team members are informed.
• Work with the interdisciplinary team to set goals and develop intervention plans aimed at optimizing patient outcomes.
• Monitor patient adherence to care plans and adjust as necessary based on progress.
• Address care gaps related to chronic diseases and surgical episodes.
• Act as the primary contact for patients regarding their conditions and facilitate access to services.
• Advocate for patients and families, addressing any questions or concerns that arise.
• Assist in managing transitions of care, ensuring effective communication and planning.
• Identify barriers to care and work towards solutions in collaboration with other care teams.
Qualifications:
• Graduate from an accredited nursing program.
• BSN preferred.
• Current state licensure as a Registered Nurse (RN) required; specialty certification preferred.
• Basic Life Support (BLS) certification through the American Heart Association (AHA) is necessary.
Experience:
• A minimum of three to five years of nursing experience is required.
Physical Requirements:
• Must possess full range of motion and dexterity.
• Requires corrected hearing and vision within normal ranges.
• May involve exposure to communicable diseases or bodily fluids.
Work Environment:
• The role may require light physical activity, including lifting and moving objects as needed.
