LPN - Patient Care Coordinator

We are seeking an experienced LPN to join our team as a Patient Care Coordinator. In this role, you will be responsible for coordinating patient care services, including scheduling appointments, collecting lab samples, and communicating with patients and families.
You will work closely with our nursing staff to ensure that patients receive the best possible care, and you will be responsible for maintaining accurate records and reporting any concerns or issues to management.
To be successful in this role, you will need to have excellent organizational skills, a strong attention to detail, and the ability to work effectively in a fast-paced environment.
Benefits:
- $60,000 - $75,000 per year, depending on experience
