The Palliative Care Nurse Practitioner/APRN delivers direct care to patients and families through specialized palliative care consultations, ensuring that patients with serious illnesses and a prognosis of one year or less receive appropriate services, particularly when they are not yet ready or eligible for hospice care.

  • Exhibits effective and ethical application of recognized evidence-based palliative medicine practices in all aspects of patient care, considering physical, emotional, spiritual, psychosocial, ethical, educational, safety, and related criteria, tailored to the ages of the patients served.
  • Demonstrates clinical proficiency by integrating the palliative care knowledge base to develop an effective care plan.
  • Systematically collects relevant information from diverse sources, including medical records, family history, and caregivers, along with comprehensive physical, emotional, psychosocial, and spiritual assessments.
  • Conducts thorough assessments utilizing palliative care domains, interprets findings, performs medication reconciliation, formulates problem-related diagnoses, initiates care plans with therapeutic interventions, and evaluates the outcomes of these actions for satisfactory responses.
  • Possesses RN/APRN licensure in the state of practice.
  • Qualified as a Nurse Practitioner/CNS with prescriptive authority, maintaining all licensure requirements.
  • Holds a minimum of 5 years of clinical nursing experience, with at least 1 year as an APRN in Hospice, Palliative Care, or Oncology preferred.
  • Advanced practice palliative care certification (ACHPN) is preferred, with consideration for certification in an appropriate advanced practice area. BCLS licensure is required. ACHPN certification is encouraged within 2 years of employment or sooner if experience requirements are met.
