We are currently seeking an experienced Occupational Therapist to join our home health team. As an Occupational Therapist at Nuvance Health, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Your Role

You will work closely with patients, caregivers, and physicians to develop, implement, and evaluate treatment plans. Your expertise will enable patients to achieve optimal independence and quality of life. You will also maintain accurate records, participate in case management conferences, and provide education and counseling to patients and families.

Responsibilities

  1. Develops, implements, and evaluates treatment plans in partnership with patients, caregivers, and physicians.

  2. Provides direct occupational therapy services in accordance with treatment plans.

  3. Maintains accurate records of patient visits, including evaluations, treatment plans, and client responses.

  4. Participates in case management conferences to coordinate occupational therapy services with overall patient care.

  5. Provides education and counseling to patients and families on occupational therapy treatment regimens.

  6. Identifies patient and family needs for other home health services and makes referrals as necessary.

  7. Supervises care provided by certified occupational therapy assistants (COTAs) and home health aides as required.

  8. Re-evaluates patient progress and treatment plans, making adjustments as needed under physician guidance.

  9. Fulfills compliance responsibilities related to the position, including in-service training.

Requirements

  • Minimum one year of experience as an Occupational Therapist
  • Demonstrated expertise in qualified home health agency settings
