SSM Health is seeking a skilled and compassionate Hospice and Palliative Medicine Nurse Practitioner to join our team. As a Nurse Practitioner in this role, you will provide high-quality, patient-centered care to individuals with serious illnesses, focusing on symptom management and improving quality of life.

Responsibilities

  • Assess and manage the physical, emotional, and spiritual needs of patients with serious illnesses
  • Develop and implement individualized care plans to address patient goals and preferences
  • Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions
  • Provide education and support to patients and their families on disease management, symptom control, and end-of-life care
  • Participate in quality improvement initiatives to enhance patient outcomes and experiences

Requirements

  • Master's degree in Nursing or related field
  • Current licensure as a Nurse Practitioner in the state of Wisconsin
  • Experience in hospice and palliative care or a related field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
