Key Responsibilities
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
  • Provide skilled nursing care, including wound care, infusion therapy, and catheter management, in patients' homes.
  • Conduct thorough assessments, identify patient needs, and develop measurable goals and outcomes.
  • Collaborate with primary caregivers to ensure optimal functioning and provide education on patient-specific needs.
  • Coordinate care with healthcare teams, including physicians, therapists, and other caregivers, to ensure seamless transitions and continuity of care.
Requirements
  • Active RN license in the state of application.
  • Valid CPR certification.
  • 1-year RN experience in a healthcare setting, preferably in home health or skilled nursing.
Preferred Qualifications
  • Medicare Skilled Nursing experience.
  • Basic understanding of Oasis.
