Physical Therapist

We are seeking a skilled Physical Therapist to join our team at Total Care Therapy. As a Physical Therapist, you will be responsible for providing high-quality patient care and rehabilitation services in a home and assisted living setting.
Key Responsibilities:- Perform treatment visits, 30-day reassessment visits, recertifications, and discharges.
- Travel to facilities or residential locations to conduct patient care.
- Identify target lists, short-term goals, long-term goals, and treatment plans.
- Complete subsequent follow-up visits.
- Provide supervision to Physical Therapist Assistants (PTAs).
- Maintain a weekly caseload of 27-32 visits (full-time).
- Utilize a universal electronic documentation system.
- Provide, recommend, and fabricate adaptive devices or other equipment.
- Educate clients, family, and staff on the use or application of such equipment.
- Licensed Physical Therapist.
- CPR/BLS Certified.
- Must be willing to travel.
- Valid Driver's License and Auto insurance Coverage.
- Reliable transportation.
- Home health experience preferred.
- Medical, dental, and vision health insurance.
