Job Summary

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our Patient Aligned Care Team (PACT) Outpatient team at the Veterans Affairs, Veterans Health Administration. As a PACT Outpatient Nurse, you will play a critical role in providing high-quality, patient-centered care to our nation's veterans.

Key Responsibilities
  • Assess, plan, implement, and evaluate care for patients with complex primary care needs.
  • Provide direct patient care, including administering medications and procedures, and collaborating with interdisciplinary teams to coordinate care.
  • Develop and implement health education and coaching programs to promote wellness, disease prevention, and chronic care management.
  • Collaborate with colleagues to develop, implement, and evaluate policies, procedures, and guidelines to improve patient outcomes.
  • Provide constructive feedback and support to colleagues to promote a safe and supportive work environment.
Requirements
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth of the United States, or the District of Columbia.
  • Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE).
  • 1-2 years of primary care, outpatient, or med-surg experience preferred.
