We are seeking a skilled Nursing Supervisor to join our team at Rosewood Rehabilitation & Nursing Center.

Job Summary

The successful candidate will be responsible for coordinating and ensuring the effective use of personnel to provide required resident care. This includes offering nursing care when particular expertise is needed.

Key Responsibilities
  • Work with the interdisciplinary Care Plan Team to conduct comprehensive assessments and develop individualized care plans for assigned residents.
  • Conduct initial and ongoing Minimum Data Set (MDS) assessments in a timely and accurate manner.
  • Utilize current physical assessment skills and knowledge of nursing principles to assess and monitor residents according to their clinical status and needs.
  • Assess residents for restorative potential and ensure that restorative programs are consistently carried out by staff to maintain residents' maximum practicable level of function.
  • Provide written and/or oral status reports of residents to ensure necessary follow-up actions.
  • Requisition and arrange for diagnostic and therapeutic services as ordered by the physician.
  • Prepare, administer, and document medications as ordered by the physician and in accordance with medication administration standards and policies.
  • Comply with Controlled Substances policy and procedures in maintaining accurate inventories and controlled logs.
  • Monitor residents and take appropriate action for side effects and adverse reactions to medications.
  • Adhere to all infection control policies and procedures.
  • Monitor and ensure that all treatments are administered as ordered and documented in accordance with policies and procedures.
  • Maintain accurate resident care records and document pertinent data reflecting the use of the nursing process, the care provided, and resident/family response to care.
  • Receive and transcribe physician's orders accurately to resident charts and treatment/care plans as required.
  • Admit, transfer, and discharge residents as required, reviewing admissions within 24 hours and ensuring paperwork is complete.
  • Ensure rooms are ready for new admissions.
  • Be visible on the units and available to assist/oversee.
  • Notify the resident's physician and responsible party when there is a change in the resident's condition and as required by facility regulations.
  • Provide direct nursing care as necessary.
  • Give an informative report to nursing staff.
  • Monitor medication passes and treatments to ensure that medications and treatments are being administered and as ordered and scheduled.
  • Ensure that all residents are checked frequently.
  • Meet with residents and families as necessary.
  • Continually review and revise the resident's care plan to ensure it represents the resident's current needs and functional status.
  • Ensure that medical and nursing care is administered in accordance with the resident's wishes, including the implementation of advance directives.

Requirements:

A graduate of an approved nursing program for registered nurses. Current Registered Nurse License in the state of Pennsylvania, current CPR certification.

Salary: $75,000 per year

Rosewood is located on a public bus route.
