Responsibilities
  • Conduct thorough patient assessments, collaborating with the care team to develop and implement tailored plans of care.
  • Makes referrals to other disciplines as indicated by patient needs or documents rationale for not doing so.
  • Promotes patient independence through teaching and rehabilitative measures, assisting patients in learning self-care techniques.
  • Supervises Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs).
  • Ensures timely and accurate documentation of patient progress and care provided.
  • Regularly communicates patient updates to clinical managers and care teams.
  • Develops and provides staff education to enhance knowledge and skills.
  • Participates in on-call responsibilities and services to patients/families as assigned.
  • Takes part in ongoing clinical development and continuing education programs.
Requirements
  • One year of clinical experience as a Registered Nurse (RN) required; less than 1 year may be approved by VP Clinical upon meeting certain criteria.
  • Current RN license specific to assigned states and CPR certification are essential.
  • A valid driver's license, reliable transportation, and liability insurance are also required.
