CenterWell Home Health is seeking a skilled Speech Language Pathologist to join our team in providing high-quality care to patients in their homes. As a Speech Language Pathologist, you will play a vital role in evaluating, directing, and providing speech/language pathology services to patients, helping them to regain their independence and improve their quality of life.
Key Responsibilities- Evaluate patients' speech and language abilities and develop personalized treatment plans to address their needs.
- Provide speech and language therapy to patients in their homes, using a variety of techniques and strategies to help them improve their communication skills.
- Collaborate with other healthcare professionals, including nurses, physical therapists, and occupational therapists, to ensure comprehensive care for patients.
- Document patient progress and communicate with patients, families, and healthcare teams to ensure seamless care coordination.
- Participate in care coordination activities and discharge planning to ensure patients receive the support they need to transition to the next level of care.
- Meet the education and experience requirements for Certification of Clinical Competence in Speech-Language Pathology or Audiology granted by ASHA.
- Minimum of six months experience as a speech therapist/speech-language pathologist.
- Home Health experience a plus.
- Current and unrestricted license.
- Current CPR certification.
- Good organizational and communication skills.
- Valid driver's license, auto insurance, and reliable transportation.
