Job Summary

We are seeking a highly skilled and experienced professional to join our team as a Heart Failure Coordinator at Mercy Hospital. This is a critical role that requires strong leadership and coordination skills to oversee various components of the STEMI Program.

Key Responsibilities
  • Monitor and address STEMI Core measure performance to ensure high-quality patient care.
  • Play an integral role in acquiring STEMI Center Certification and other quality processes.
  • Develop and maintain a method to identify hospitalized patients with a primary or secondary diagnosis of heart failure and are at high risk for readmission.
  • Positively articulate the vision of the heart failure program to all members of the care team to influence organizational change.
Qualifications
  • Licensed Registered Nurse in the State of Missouri.
  • Five (5) years of experience in Healthcare/Medical - Cardiac Services.
  • BSN required, current RN licensure, and 5 years of experience in Cardiology Nursing.
