Job SummaryWe are looking for a highly skilled and compassionate Nurse Practitioner to provide primary care to our residents. As a key member of our medical team, you will be responsible for assessing patient health, performing physical examinations, and developing evidence-based care plans.
Responsibilities- Provide primary care to long and short-term residents, including assessing patient health and developing care plans.
- Collaborate with the medical attending/physician and nursing home staff to provide high-quality care.
- Build relationships with residents and families, educating them on their care and involving them in the decision-making process.
- Comply with federal, state, and local regulations, as well as maintain professional and technical knowledge through ongoing education and training.
- Maintain a safe and clean working environment, adhering to the organization's core values of respect, professionalism, and cultural sensitivity.
- Board certification as a Nurse Practitioner.
- Experience in a hospital setting, with a focus on long-term care.
