Job SummaryWe are seeking an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to join our team on a full-time basis. As a primary care provider, you will be responsible for delivering high-quality care to homebound and palliative care patients in the Silver Spring County, MD region. You will work under the supervision of a physician and care for a diverse set of patients with chronic diseases.
Key Responsibilities- Provide high-quality care to primary and palliative patients in and around Silver Spring County.
- Assess patients' health by performing physical examinations, obtaining and updating medical histories, and creating personalized care plans.
- Effectively communicate with office staff and clinical operations across various internal platforms.
- Provide patient-centered care to a panel of patients, developing impressions, diagnoses, and treatment plans.
- Foster trust and consult with patients, families, and caregivers during visits.
- Order diagnostic testing as medically necessary and prescribe medications, supplies, and interventions.
- Board Certified, with a valid NP or PA certification in the state of Maryland.
- Strong clinical skills with the ability to accurately and confidently treat, assess, and diagnose.
- Positive demeanor and strong people skills.
- Ability to confidently problem-solve and operate independently.
- 1-2 years of primary care experience with geriatric patients (hospice experience highly preferred but not required).
- A flexible schedule to accommodate your needs.

- $105,000-$140,000+ (depending on experience and other incentives) (full-time).
