Key Responsibilities:
- Provide high-quality primary care services to patients of all ages
- Conduct thorough health assessments and develop individualized treatment plans
- Order and interpret diagnostic tests, prescribe medications, and provide education to patients
- Collaborate with healthcare teams to ensure seamless patient care
Requirements:
- Graduate of an accredited Nurse Practitioner Program and certified by the American Nurses Credentialing Center or the American Academy of Nurse Practitioners
- Permanent and unrestricted license to practice as a nurse practitioner in Arizona
- Current board certification in family medicine and maintenance of certification
- Current medication dispensing license (DEA)
- At least 1 year of experience in a family practice or comparable setting, with experience in a community health center setting preferred
- Current ACLS certification
