Job Summary:

HealthMark Rehabilitation is seeking a part-time Speech-Language Pathologist (SLP) to join our team at Park Manor of Tomball facility in Tomball, Texas. As an SLP, you will provide therapy services to residents and short-term patients, developing and implementing individualized treatment plans to meet their unique needs.

Responsibilities:

  • Provide speech and language therapy services to patients in accordance with physician's orders and accepted professional practices.
  • Develop and implement individualized treatment plans to address patients' communication and swallowing disorders.
  • Conduct assessments and evaluations to identify patients' strengths and weaknesses, and develop goals and objectives to address their needs.
  • Collaborate with interdisciplinary team members to ensure comprehensive care and coordination of services.
  • Document patient progress and outcomes in accordance with regulatory requirements and company policies.
  • Participate in quality improvement initiatives to enhance patient care and services.

Requirements:

  • Current Texas license/certification for Speech-Language Pathology (CCC-SLP).
  • Strong oral and written communication skills, with ability to interact effectively with patients, families, and healthcare team members.
  • Organizational and analytical skills, with ability to prioritize tasks and manage time effectively.
  • Ability to work independently and as part of a team, with strong problem-solving and decision-making skills.
  • Meet physical requirements of the job, with ability to lift, bend, and move patients as needed.
