Job Summary

The Case Manager facilitates the transition of patients from the acute area setting to the next appropriate level of care, working closely with physicians and payors to ensure proper use of fiscal resources.

This role requires a strong understanding of nursing principles and practices, as well as excellent communication and problem-solving skills.

Requirements
  • Graduate from a CCNE or NLN accredited nursing program
  • Four years related experience and/or training; or equivalent combination of education and experience
  • Licensed as a Registered Nurse in the State of South Dakota and/or meeting the requirements of Multi-state licensure
