Position Overview:
As an Outreach and Enrollment Navigator at TAPM, you will be instrumental in assisting patients in navigating the healthcare landscape, helping them secure healthcare coverage, and linking them with community resources to address obstacles to care. Your key responsibilities will encompass:
- Guiding patients through sliding fee applications, Medicaid enrollment, and ACA open enrollment periods
- Educating patients about insurance options and available healthcare services
- Identifying and connecting patients with resources for financial aid, transportation, and housing support
- Engaging in community outreach initiatives to inform patients about available services
- Collaborating with the healthcare team to enhance patient outcomes
Essential Qualifications:
- 1 to 3 years of experience in healthcare, community outreach, or similar fields
- Bilingual skills are preferred (English/Spanish)
Skills and Training:
We welcome candidates who possess proficiency in Microsoft Office, strong communication abilities, and the capacity to work autonomously. Additional qualifications include:
- Exceptional organizational and problem-solving capabilities
- Familiarity with electronic documentation systems
