Role Summary: As a vital member of our healthcare team, the Nurse Practitioner will be responsible for delivering high-quality medical care to our participants, ensuring their health and well-being through comprehensive assessments and tailored care plans.
Key Responsibilities:
- Conduct thorough in-person evaluations, including initial assessments, routine check-ups, and urgent evaluations to provide continuous medical support.
- Collaborate closely with physicians and the Interdisciplinary Team (IDT) to formulate and implement individualized care strategies for each participant.
- Engage in discharge planning alongside physicians and IDT to facilitate smooth transitions from acute or long-term care settings.
- Provide ongoing support to participants residing in nursing facilities, ensuring regular visits based on individual needs.
- Work in partnership with client physicians, community-based PACE physicians, and IDT to ensure seamless care coordination.
- Under the guidance of the Medical Director, analyze patient outcomes and resource utilization data to identify best practices and enhance policy frameworks.
- Participate in a rotating schedule for night and weekend on-call duties.
Qualifications:
- Graduation from an accredited nursing program.
- Certification as a Nurse Practitioner with Prescriptive Authority.
- Valid Nurse Practitioner State License.
- A minimum of two years of experience working with elderly populations.
- Up-to-date knowledge of chronic care and geriatric best practices.
- Possession of a valid State Driver's License with a clean driving record.
- Proof of current vehicle insurance is required.

Required Skills and Certifications:
- Certification as a Nurse Practitioner with Prescriptive Authority.
- Licensure in California.
- Experience in geriatrics is preferred.
- Minimum of two years of Nurse Practitioner experience.
