Key Responsibilities- Assess patient physical, psychological, social, spiritual, educational, developmental, cultural, and discharge planning needs.
- Develop and implement individualized plans of care in collaboration with the multidisciplinary team.
- Provide direct patient care, coordinate care with other healthcare team members, and delegate tasks as needed.
- Evaluate patient progress and adjust plans of care accordingly.
- Document patient information accurately and thoroughly.
- A current, valid RN license in the state of practice.
- At least one year of experience in an acute care hospital setting.
- Ability to work effectively in a fast-paced environment and prioritize tasks accordingly.
