School-based Health Center Role
KEY RESPONSIBILITIES
- Treat all individuals, including staff, practitioners, visitors, and patients, with utmost dignity and respect.
- Conduct patient interviews to gather medical history, measure vital signs, and perform vision screenings, ensuring accurate documentation in the Electronic Health Record (EHR).
- Collaborate with team members by sharing patient information to enhance comprehensive care.
- Assist healthcare practitioners during examinations as required.
- Administer medications and injections according to practitioner directives.
- Ensure proper sterilization and maintenance of medical equipment.
- Collect laboratory specimens and conduct routine in-house laboratory tests.
- Prepare specimens for external laboratory analysis.
- Perform basic wound care as necessary.
- Maintain cleanliness and adequate supplies in examination rooms.
- Adhere to clinic protocols and contribute to the development of new guidelines.
- Handle clerical tasks, including paperwork, phone communications, and data entry.
- Conduct routine diagnostic tests (e.g., EKG) and schedule patients for external diagnostic procedures.
- Engage in community health initiatives and disease prevention efforts.
- Participate in the organization’s Quality Improvement Program.
- Attend daily team meetings with staff as required.
- Contribute to staff development programs and foster team collaboration.
- Manage personal and team clinical inboxes effectively.
- Ensure prompt responses to appointment inquiries via phone and online portals.
- Educate patients about the Patient-Centered Medical Home (PCMH) model and available care team resources.
- Actively participate as a member of the patient care team.
- Provide patients with a copy of their visit summary.
- Assist patients in scheduling follow-up appointments and addressing transportation concerns.
- Ensure patients have a designated primary care provider.
- Perform additional duties as assigned.
QUALIFICATIONS
Completion of an accredited medical assisting program, along with at least one year of experience in the healthcare field or an approved internship program, or an equivalent combination of education and experience.
