The Licensed Practical Nurse (LPN) Clinical Care Leader (CCL) is a key member of our healthcare team, responsible for providing high-quality patient care in a long-term care setting. As a CCL, you will work closely with our interdisciplinary team to develop and implement individualized care plans, ensuring the best possible outcomes for our patients.
Responsibilities- Assess patient needs and develop care plans in collaboration with the healthcare team
- Provide direct patient care, including administering medications, wound care, and vital sign monitoring
- Collaborate with other healthcare professionals to ensure seamless care transitions and effective communication
- Document patient information accurately and thoroughly, maintaining confidentiality and adhering to HIPAA guidelines
- Participate in quality improvement initiatives and contribute to a culture of excellence in patient care
- Graduate of an accredited practical nursing program
- Current LPN licensure in the state of practice
