Job Responsibilities
  • Provide high-quality patient care in post-acute facilities, working closely with residents and healthcare teams to ensure optimal outcomes.
  • Monitor vital signs, administer medications, and document health concerns and medical history.
  • Participate in IV set up, immunizations, and phlebotomy procedures, and assist with the cultivation of a safe and comfortable facility environment.
  • Collaborate with medical staff to develop and implement resident care plans, and maintain accurate records and reports.
Qualifications and Skills
  • Current Massachusetts state license to practice as a Licensed Practical Nurse (LPN).
  • Completion of an approved LPN educational program.
  • High school graduation, GED, or equivalent.
  • At least 6 months of experience in a similar role.
  • Ability to lift a minimum of 50 lbs.
