Job Title: LPN - Licensed Practical Nurse

Job Summary:

We are looking for a highly motivated and experienced LPN to provide exceptional patient care in post-acute facilities. As an LPN with IntelyCare, you will have the opportunity to work with a diverse range of patients and develop your skills in a dynamic and supportive environment.

Responsibilities:
  • Provide compassionate nursing care to patients in post-acute facilities
  • Monitor patients' physical and mental conditions and track vital signs
  • Perform resident intakes, document medical histories, and track health concerns
  • Administer and verify medications, track use and effects
  • Cultivate a comfortable and safe facility for residents
  • Track medical records and charts
  • Assist with IVs, immunizations, and phlebotomy procedures
  • Manage medical supplies and maintain inventory
  • Adhere to resident care plans and follow facility protocols
Requirements:
  • Current Pennsylvania state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of approved LPN educational program
  • High school diploma or GED (or equivalent)
  • Ability to lift a minimum of 50 lbs
  • Minimum of 6 months of experience (LPN or related role)
