Responsibilities

  • Implement and manage patient care plans in collaboration with physicians and other healthcare professionals
  • Conduct patient assessments and coordinate care to ensure optimal health outcomes
  • Administer prescribed medications, treatments, and therapies as ordered by physicians
  • Document patient care and progress in a timely and accurate manner
  • Provide education and training to patients and their families on health promotion and disease management
  • Collaborate with the care team to ensure seamless transitions of care

Requirements

  • Active, unencumbered license as a Licensed Vocational Nurse (LVN) in the state of practice
  • Current CPR certification from American Red Cross or American Heart Association
  • TB skin test (current within the last 12 months)
  • At least six months of prior hands-on nursing experience
  • Reliable transportation
