Key Responsibilities:- Provide direct patient care in the home setting, utilizing strong critical thinking skills and effective communication with patients and their families.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans that address the unique needs of each patient.
- Conduct thorough assessments and evaluations to identify patient needs and develop strategies for improving patient outcomes.
- Participate in ongoing education and training to stay current with best practices in home health care and to enhance clinical skills.
- 1 year of experience in acute care, rehabilitation, or homecare.
- Valid NYS RN and DMV licensure.
- A comprehensive orientation to home health care, including training on our electronic medical record system and other essential tools.
- A supportive work environment with access to clinical nurse specialists, in-depth computer training, and assistance with insurance authorizations.
- The opportunity to work independently, starting and ending your day from home, and to experience the rewards of appreciation from patients and families.
Pay is based on experience, skills, and education, with a competitive hourly rate. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
