Job Summary

The Private Duty Nurse LPN will provide one-on-one care to patients in their homes, assisting with daily living activities, administering medications, and providing wound care. The ideal candidate will have a strong commitment to delivering high-quality patient care and a passion for working in a home health setting.

Responsibilities
  • Provide one-on-one care to patients in their homes, including assistance with daily living activities, administering medications, and providing wound care.
  • Assess patients' needs and develop individualized care plans to meet their unique needs.
  • Collaborate with patients' healthcare teams to ensure seamless care coordination.
  • Document patient care and progress in a timely and accurate manner.
  • Participate in ongoing education and training to stay current on best practices in home health care.
Requirements
  • Current LPN licensure in the state of employment.
  • At least 6 months of experience in home health care or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility to work a variety of shifts, including days, evenings, and weekends.
