Nurse Care Specialist

BAYADA Home Health Care seeks a compassionate and dedicated Registered Nurse (RN) for a full-time Hospice position in Middlebury, Vermont.

Responsibilities- Deliver high-quality patient care through regular home visits, assessing client needs, and developing personalized care plans.
- Collaborate with a multidisciplinary team to provide comprehensive palliative care, ensuring clients receive the medical attention required to remain at home.
- Perform various clinical duties, including medication administration, wound care, treatments, and procedures, as well as comfort care.
- Monitor clients' conditions, reporting any changes to the Clinical or Client Services Manager.
- Follow up on doctors' orders, execute, and properly document them.
- A current license as a Registered Nurse (RN) in Vermont.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care and/or hospice experience is strongly preferred but not required.
- Solid computer skills and prior experience with electronic medical records (EMR) are beneficial.
- Ability to travel to cases as assigned.
The estimated salary for this role is around $85,000 per annum, reflecting the company's commitment to rewarding its employees for their exceptional service.
