Key Responsibilities- Provide direct patient care under the supervision of our medical team, ensuring the highest level of quality and safety.
- Collect and document patient data, including vital signs, medical history, and treatment plans.
- Administer medications and treatments as prescribed by our medical team.
- Perform routine medical procedures, such as venipuncture and blood glucose testing.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Maintain accurate and up-to-date patient records, including medical history, treatment plans, and test results.
- Collaborate with our medical team to develop and implement individualized care plans.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Graduate of an accredited school of Practical Nursing.
- Currently registered in New York State as a Licensed Practical Nurse (LPN).
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Strong attention to detail and organizational skills.
- Three years' experience in ambulatory care setting as a LPN.
- Experience with electronic medical records and practice management systems.
