Position Summary
We are seeking a qualified Supportive Care Supervisor. This full-time role comes with a comprehensive benefits package, including Medical, Dental, Vision Insurance, Paid Time Off, and a 401k plan.
Key Responsibilities
- Oversee and coordinate social work services for PACE participants and their families under the guidance of the Operations Director.
- Conduct social work assessments and develop treatment plans for participants.
- Provide education and counseling to participants, caregivers, and family members to enhance community support.
- Engage in individual participant interactions and maintain appropriate collateral contacts.
- Assist in resource identification and address mental health needs as they arise.
- Manage ongoing casework and advocate for participant and caregiver needs.
- Facilitate communication between the interdisciplinary team, caregivers, and community organizations.
Qualifications
- Master's degree in Social Work from an accredited institution.
- Certification or licensure from the State Board of Social Work.
- Minimum of two years of experience in a collaborative team environment within healthcare settings.
- Experience with elderly or frail populations is preferred; training will be provided if necessary.
- Possession of reliable transportation, a valid driver's license, and the required state auto insurance.
- Must be medically cleared for communicable diseases and have current immunizations before direct participant interaction.
- Successful completion of a comprehensive background check.
Skills and Competencies
- Understanding of the psycho-social challenges faced by frail and chronically ill individuals and their caregivers.
- Ability to conduct psychosocial assessments and provide individual, family, and group counseling.
- Proficient in maintaining accurate documentation and preparing clear reports.
- Strong public speaking skills for diverse audiences.
- Excellent verbal and written communication abilities.
- Proven capacity to work effectively within an interdisciplinary team.
- Competency in computer applications.
