Medical Professional for Complex Patient Care

About the RoleWe're seeking an experienced Medical Professional to join our team in Middlesex County, CT. As a key member of our Home and Community team, you'll work closely with patients, family members, and caregivers to develop and implement personalized care plans that meet their unique needs.
Responsibilities- Conduct comprehensive assessments of newly enrolled patients and provide ongoing care and support
- Develop and update patient management care plans to ensure optimal outcomes
- Collaborate with patients, family members, and caregivers to educate them on disease processes, treatment options, and wellness strategies
- Prioritize and coordinate care activities to ensure timely and effective delivery of services
- Participate in ongoing quality improvement initiatives to enhance patient care and satisfaction
- Graduate of an accredited Nurse Practitioner or Physician Assistant program
- Current state RN/NP or PA license
- Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or National Commission on Certification of Physician Assistants (NCCPA)
- Access to reliable transportation and ability to pass a motor vehicle safety test
- 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care
- Familiarity with long-term care, community resources, and cost-effective alternatives for patient care delivery systems
- Knowledge of CPT, ICD-10, and HCPC codes
- Awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems
The salary range for this role is $88,000 to $173,200 annually, based on experience, education, and certifications. In addition to your salary, UnitedHealth Group offers a comprehensive benefits package, incentive programs, equity stock purchase opportunities, and 401(k) contribution. This role requires a minimum of 12 months in an active regular status; after which, employee must remain in the Advanced Practice Clinician or Physician role within Home and Community for 36 months to receive the full benefit of student loan repayments.
