Your RoleAs a CPN, you will play an essential role in partnering with the medical director to engage and educate hospitalist physicians and advanced practice providers on quality and process improvement initiatives. Utilizing key metrics from our proprietary software, you will measure and track performance outcomes effectively.

This multifaceted position within the hospitalist team does not involve direct patient care; however, you will engage in daily workflows, advocate for patient objectives within care plans, and ensure seamless communication throughout the care continuum as patients navigate their hospital experience. Our commitment to enhancing healthcare quality extends beyond patient discharge, emphasizing the importance of continued care.
As a CPN, you will work a standard 40-hour week, Monday through Friday, in a fast-paced setting, maintaining meticulous attention to detail and accuracy. This role demands strong clinical expertise and experience in an acute care hospital environment, with the ability to assess patient clinical status and readiness for discharge being crucial.
Qualifications:
- Current RN licensure in good standing; a Bachelor of Science in Nursing (BSN) is preferred.
- A minimum of two years of nursing experience is required. Additional experience in case management, quality improvement, utilization review, transitional care, or post-acute services is advantageous but not mandatory.
- Familiarity with quality improvement initiatives related to patient satisfaction, length of stay, and readmission reduction is beneficial but not essential.
- Strong critical thinking skills and proficiency with technology, including electronic medical records and Microsoft Office.
