The PRN RN/LPN at Elite Home Care Day Centers & Transportation plays a crucial role in delivering exceptional care to our members on an as-needed basis. This position is integral to our day center team, focusing on enhancing the physical, emotional, and mental health of our members. This is a PRN role, available Monday through Friday, covering our current and upcoming locations.
Key Responsibilities- Administer member care, including medication management and nursing treatments as per medical directives.
- Monitor and assess members' health, promptly reporting any changes or concerns to the Administrator.
- Provide education to members regarding their health conditions, treatment options, and preventive health measures.
- Maintain accurate and current member records, developing and implementing care plans upon enrollment as necessary.
- Review and update care plans at least every six months, along with quarterly nursing notes on each member's status.
- Possession of a valid and current nursing license in South Carolina, in good standing.
- A minimum of one year of nursing experience, ideally with senior adults or individuals with disabilities in a community setting.
- Strong interpersonal and communication skills for effective interaction with members, families, and colleagues.
- Excellent organizational and time management abilities.
- Capability to work both independently and collaboratively within a team.
- Willingness to complete any necessary background checks and drug screenings.
