Job Summary

The LPN MDS Nurse Coordinator will be responsible for ensuring accurate and timely completion of MDS assessments and coordination of the RAI process. This role requires strong clinical knowledge, attention to detail, and excellent communication skills.

Key Responsibilities
  • Schedule and complete MDS assessments per RAI guidelines, coordinating with the interdisciplinary team as needed.
  • Develop comprehensive resident care plans with the interdisciplinary team, ensuring that all care area triggers are addressed.
  • Review and update care plans quarterly, reflecting current resident status and individualized problems, goals, and interventions.
  • Coordinate with other disciplines to ensure timeliness of assigned sections of MDS completion and report noncompliance to the Executive Director.
  • Sign off on MDS sections V0200B comprehensive CAA completion and Z0500A MDS completion.
  • Oversee clinical reimbursement and case management services within the center for Medicare A and B, managed care, insurance, and Medicaid (case mix).
  • Direct and oversee the implementation of CommuniCare Family of Companies programs, policies, and procedures related to the RAI process.
  • Coordinate timely and effective service delivery for all residents while ensuring quality clinical outcomes and appropriate reimbursement.
  • Advocate on behalf of residents and the facility for needed resources and services.
  • Provide education to healthcare providers, the care team, and residents/families on Medicare, Medicaid (case mix), managed care, and MDS completion.
  • Liaise between the facility, residents/families, rehabilitation, and other care providers.
  • Oversee and monitor MDS documentation and charting requirements supporting services provided to meet billing requirements, including state-specific requirements.
  • Provide oversight of Medicare and managed care residents, coordinate rehabilitation and optimal recovery, and assist with discharge planning.
  • Oversee validation of Medicare entitlement and eligibility for prospective residents.
  • Complete chart audits to assess the quality of documentation supporting skilled Medicare, managed care, and Medicaid coverage.
  • Ensure compliance with federal and state regulations, as well as CommuniCare Family of Companies' policies and procedures regarding state-specific case mix.
Requirements
  • Graduate of an accredited school of nursing; LPN license in the state employed.
  • Three years of experience in a long-term care environment preferred.
  • Experience with the MDS/RAI process and/or case management preferred.
