Institute on Aging is seeking a highly skilled and experienced Home Health Clinical Manager to join our team. As a key member of our Leadership Team, you will be responsible for the day-to-day leadership, administration, and oversight of operations related to home care and home health services.

Key Responsibilities:
  • Manage and oversee the daily operations and services provided by the home care team, including staffing, care planning, quality of care, participant satisfaction, and program compliance.
  • Oversee the delivery of home health services, including operationalizing and overseeing compliance with the home health license and coordinating with home health contracted vendors.
  • Work closely with clinic leadership to ensure close communication and effective coordination of medical care.
  • Ensure the department has adequate staffing, oversee schedules, and adhere to a competency program.
  • Responsible for staff development, training, recruiting, retention, and employee engagement.
  • Monitor and facilitate program compliance with federal, state, local, and plan-level requirements.
  • Ensure best clinical practices and that nurses and caregivers are performing within their full scope and providing consistency and continuity of care.
  • Responsible for building a highly effective home care department that fully integrates custodial home care and home health services into the overall care plan.
  • Ensure clarity of procedures, standards of care, and collaboration within and between other departments.
  • Adhere to SEIU 2015 terms and conditions and participate in collective bargaining, management meetings, or other Union meetings as needed.
  • Promote quality of care by participating in Quality Assurance Improvement Plan (QAIP) activities, initiatives, process improvement plans, committees, etc.
  • Provide clinical leadership through oversight of infection control best practices.
  • Monitor and help develop budget for areas of responsibility.
  • Participate in after-hours rotational on-call duties.
  • Actively participate in and encourage actions that promote good public relations with participants, their families, and friends, visitors, and the community.
Requirements:
  • Active California Registered Nurse license.
  • Graduate of an accredited college or university with a baccalaureate degree in nursing.
  • Three years of management experience in a healthcare setting.
  • At least one year of experience with frail older adults.
  • Familiarity with Title 22 regulations preferred.
  • Experience driving home health licensure efforts and participating in audits.
  • Demonstrated skills leading and motivating teams and individuals.
  • Understanding of the importance of relationship building.
  • Familiarity with the application of Information Technology (IT) to clinical care, including patient care record-keeping, quality assurance, outcomes assessment, and pertinent privacy requirements.
  • Ability to work within an interdisciplinary team with initiative, imagination, resourcefulness, and flexibility.
  • Interest and ability to work in a multilingual, multicultural environment.
  • Bi-lingual Cantonese a plus.
Physical Requirements:
  • Ability to lift, transfer, push/pull, maneuver, and reposition 35 lbs.
  • Ability to reach, bend, and walk.
  • Five-finger manipulation.
Compensation:

$128,000 - $436,160 USD Annual

This amount is not necessarily reflective of actual compensation that may be earned, nor a promise of any specific pay for any specific employee, which is always dependent on actual experience, education, and other factors.

This range does not include any additional equity, benefits, or other non-monetary compensation which may be included.
