Job Summary

We are seeking a highly skilled Anticoagulation Pharmacist to join our team at Catholic Health Initiatives. As a key member of our pharmacy team, you will be responsible for managing patients on warfarin and ensuring their medications are safe and effective.

Responsibilities
  • Review prescriptions for safety and accuracy, checking for potential interactions and allergies.
  • Collaborate with interdisciplinary teams to provide information and support patient care.
  • Provide exceptional service to patients and nurses, ensuring timely and accurate medication fulfillment.
  • Participate in quality improvement initiatives to optimize patient outcomes.
Requirements
  • Pharmacist License
  • Strong clinical skills and knowledge of anticoagulation therapy
  • Excellent communication and teamwork skills
What We Offer

Competitive hourly rate: $46.74 - $67.77
