Our Subacute Care Center (SNF) offers comprehensive 24-hour support for adult and elderly patients recovering from surgery or managing various medical conditions.

Patients in this unit typically transition from an acute care setting and require a short-term, intensive nursing or rehabilitation program to facilitate their recovery and prepare them for discharge.

As a Patient Care Nursing Assistant (PCNA), your core responsibilities will encompass providing essential patient care under the guidance of a Registered Nurse (RN), monitoring and reporting any significant changes in patients' health status, and ensuring that patient environments are clean and organized.

Key Responsibilities:

  • Monitor and report any changes in patients' physical, mental, and emotional conditions to the RN, including vital signs, weights, and assistance with personal care and daily activities.
  • Maintain cleanliness and orderliness in patient rooms.
  • Demonstrate a positive, empathetic, and sensitive approach in all interactions.
  • Exhibit strong interpersonal skills for effective communication with patients and staff.
  • Follow oral and written instructions proficiently.
  • Perform basic clinical calculations as needed.

Other duties will require demonstrating competencies across professional, technical, and clinical knowledge areas in a safe and efficient manner.

Education Requirements:
High School Diploma or GED is required.

Certifications:
State Tested Nursing Assistant (STNA) certification is mandatory.

Nursing students must obtain Basic Life Support (BLS) certification from the American Heart Association (AHA) prior to employment.


Work Experience:

Candidates should demonstrate proficiency in applied mathematics and reading comprehension at a minimum junior high level as assessed during the employment process.

Nursing students who have completed at least one clinical rotation as part of an accredited nursing program may substitute this experience for the required work experience.


Physical Requirements:

This role necessitates full-body mobility to assist and lift patients, along with manual dexterity and good eye-hand coordination; extensive standing and walking are involved.

Medium Work
  • Involves exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
Physical demand requirements exceed those of Light Work.

Adhere to standard precautions and utilize personal protective equipment as required.
