Job Summary: We are seeking a highly skilled Inpatient Clinical Pharmacist to join our team at Clinical Management Consultants. As a key member of our healthcare team, you will play a critical role in providing exceptional patient care and supporting our hospital's pharmaceutical efforts.
Key Responsibilities:
- Monitor patient progress and evaluate the appropriateness and effectiveness of medication regimens.
- Collaborate with physicians and nurses to develop and implement care plans for acute care patients.
- Assess patient health problems and identify whether medications are meeting their needs.
- Work closely with a team of healthcare leaders to create a comprehensive care plan for patients.
