Job Summary

As an Ambulatory Care Pharmacist at Kaiser Permanente, you will play a critical role in providing high-quality, patient-centered care in an outpatient setting. Under indirect supervision, you will utilize approved protocols to manage patient drug therapy, provide education, and prepare and/or oversee the preparation and dispensing of medications.

Key Responsibilities
  • Collaborate with healthcare professionals to promote rational and cost-effective drug therapy
  • Provide direction to Pharmacist Residents, Interns, and other non-licensed personnel
  • Conduct patient medication history inquiries and synthesize treatment plans
  • Identify, resolve, and prevent medication-related problems
  • Initiate, adjust, and monitor drug therapy based on patient information and laboratory monitoring
  • Provide patient education and monitoring to optimize outcomes and detect adverse drug reactions
  • Assure ongoing performance improvement of pharmaceutical care services
Requirements
  • Successful completion of a clinical pharmacy residency or equivalent experience
  • B.S. degree in Pharmacy or Pharm.D. required
  • Pharmacist License (California) required at hire
  • Basic Life Support certification from American Heart Association
  • Comprehensive knowledge of pharmacological principles, medical terminology, and related information
  • Excellent verbal and written communication skills
