Key Responsibilities- Assist patients with personal care activities, such as bathing, dressing, and grooming, as prescribed on the Home Health Aide plan of care.
- Perform light housekeeping and meal preparation tasks to support patients and their families.
- Monitor patient status and report any changes to the interdisciplinary team as needed.
- Maintain accurate and up-to-date documentation of patient care.
- Comply with policies and procedures of Hospice of Huntington, Inc.
- Protect patient confidentiality and maintain a professional demeanor at all times.
- A high school diploma or equivalent is preferred.
- Proof of nursing assistant certification is required.
- Minimum of 1 year of experience in a healthcare setting is preferred.
- Current driver's license, automobile insurance, and reliable transportation are necessary.
- Philosophy and values must be compatible with the hospice concept.
