Job Summary:

The Physical Therapist provides comprehensive physical therapy services to patients by assessing, planning, implementing, and documenting patient care and treatment programs. The therapist participates in department activities designed to enhance patient care and programs.

Responsibilities:

• Assess patients to determine physical therapy needs
• Develop and implement individualized treatment plans
• Document patient progress and treatment outcomes
• Participate in department activities to enhance patient care and programs

Requirements:

• Bachelor's, Master's, or Doctorate degree from an accredited Physical Therapy program
• Wisconsin state license as a Physical Therapist
• American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent
